YMS Health and Social Care assembled a highly skilled team of social workers, each with over five years of relevant project experience, to conduct face-to-face care reviews. The project was bolstered by a dedicated Project Excellence Lead who provided clinical support and ensured each review was thoroughly scrutinised before submission.
Throughout the project, the team demonstrated professionalism and dedication by providing regular progress updates in detailed reports, keeping Newcastle City Council consistently informed. They ensured all care plans were relevant, strengths-based, and adhered to best practice standards. Extra care was taken to identify and report any potential safeguarding issues within the visited care homes, showcasing a deep commitment to the well-being and safety of residents.
